A 22-year-old male with severe atopic dermatitis developed a new pruritic eruption on the wrists and trunk while receiving dupilumab therapy. Clinical and histopathological findings confirmed the diagnosis of lichen planus. Symptoms of atopic dermatitis improved with standard treatment, whereas the lichenoid eruption persisted.
\nDupilumab therapy was discontinued, and treatment with the selective JAK1 inhibitor upadacitinib was initiated. Dupilumab therapy may promote lichenoid reactions in susceptible individuals. The appearance of new skin lesions during biologic therapy should prompt reconsideration of the diagnosis and, when necessary, confirmation by histological examination.<\/p>\n","protected":false},"excerpt":{"rendered":"
Dupilumab is an effective biologic therapy for moderate-to-severe atopic dermatitis, but inflammatory skin reactions associated with immune shift have rarely been reported.
